Case Management

You can have the assistance of a professional to link you to services in the community such as housing, medical, educational and transportation needs.  Case Management is helpful when individuals have many needs and don't know where to start.  A Case Manager can help you identify your needs and help link you to community programs that will help meet all of your needs, not just mental health.  The goal of Case Management is to help you secure independence in your environment and community. Case Management  works to assist adults and children in gaining access to needed medical, psychiatric, social, and educational services among others. Your case manager will focus on linking you to services, coordinating with them, and advocating for you to ensure your specific needs are best met.
